Ages and Stages Questionaires & Activities

Here are a collection of questionaires and activities specific for each listed age of development for your child. Please respect the copyright for them, but use them for your own family needs. The needs may be for: reassuring yourself that your child is on track, looking ahead as to their coming developmental abilities, confirming a concern that you may have, or responding to our request for you to bring the completed form to the next scheduled appointment. Each questionaire begins with an explanation page to help you understand how to correctly apply this tool to your child's developmental progress. Choose the one that most closely matches your child's age, or the age your child will be at the scheduled checkup, and answer the questions as objectively as possible. (Ask a friend for help if necessary). Don't expect your child to have mastered all the tasks listed, and remember this is only a screening tool and is not used to make a diagnosis of Developmental Delay.
